Unfortunately, this is what healthcare looks like for many people. A precarious journey, with a small window of safety.
When caring for safety net populations, we need as many resources and tools as possible to fill the care gap so that all patients have access to healthcare.
Safety Net Providers (SNPs) are caring for the costliest patients with the least amount of resources.
Additionally, these groups have the most SDOH obstacles that present bigger challenges to overcome.
Our product serves as an effective tool for Black populations of any SNP. SNPs can prescribe our patient-facing health apps to their patient populations and integrate our Hico DashBoard into their workflow to observe patient engagement, progress and completion.
We recommend a six month pilot to demonstrate the benefits of being able to offer a mobile first, culturally sensitive virtual health program designed specifically for Black populations.
